- Background Check Form for all volunteering parents (done every 2 years, please include a self-addressed stamped envelope)
FOR ALL STUDENTS NEEDING ANY MEDICATION, PRESCRIPTION OR OVER THE COUNTER WHILE AT SCHOOL
- Oral Medications Form and medications —if your student will be taking any medication during the school day, this form must be completed by you and your physician and the medications brought with the form to the Three Points Office.
